C32012 is intended for physicians, industry members, residents, nurses, technologists, and allied health personnel. The registration fee includes course materials, attendance at all sessions and meal functions. Full payment is required to confirm registration. Industry professionals exhibiting at C3 should register directly with their company.
C3 is pleased to waive all registration fees for attendees who book at least 3 nights stay at the Rosen Shingle Creek during the conference. Once your hotel is confirmed, email your hotel confirmation to Julie Enichen at jenichen@achlcme.org to receive your complimentary coupon code.

Refunds/Cancellation Policy
Cancellations will be accepted until May 31, 2012, and are subject to a 10% administrative fee. No refunds will be given after that date. ACHL must be notified in writing of your intent to cancel prior to the cancellation deadline. Email or fax your cancellation notification to jenichen@achlcme.org or fax (773) 714-0707.
Accommodations
Please contact the Rosen Shingle Creek directly to make your hotel reservation at 866-996-6338 or www.rosenshinglecreek.com. A block of rooms at a discounted rate has been reserved for C3 participants until Monday, May 14, 2012. After that date or when the block fills, reservations will be subject to availability and the discounted room rate will no longer be valid. The discounted room rate is $175 single/double occupancy, per night, plus tax (12.5%). There is an additional charge of $20 per person for additional guests. All reservations must be accompanied by a first night room deposit or guaranteed with a major credit card. To reserve a room at the discounted rate, please contact the reservations desk at the above number and mention that you are participating in the C3 Conference.
